VA CCN Program
Tradition Home Care is now providing care for Veterans through the VA CCN program!
The VA Community Care Network (VA CCN) was designed to help veterans access medical care in their respective communities. The program was created in 2019 and replaced the traditional Veterans Choice Program. The VA CCN program makes is easier for our Veterans to get the care they need close to home without having to travel to a VA Medical Center.
Through VA CCN, Veterans can access care from a network of community providers who have been approved by the VA to provide medical services to Veterans. These providers include primary care physicians, specialists, and other healthcare professionals.
At Tradition Home Care, we are proud to be a part of the VA CCN network and offer services to Veterans in their homes.
